In order for your child to attend Lake Windward he/she will need to be registered. You can register your child at the main office at Lake Windward.


Here is the information required to register:


   1. Parent's valid driver's license.

      

   2. Proof of residency such as a utility bill, house sales agreement, or leasing agreement.

      

   3. Student's birth certificate certified by the state where the student born (we cannot accept those certified by hospitals).

      

   4. Student's social security card. (Parents may ask the school to issue a Student I.D. number instead of using the student's social security number on school records.)

      

   5. Georgia Immunization Form (3231). Those students born after Jan. 1, 1992 must also include proof that the Hepatitis B series has been started.

      

   6. Georgia Hearing, Vision, and Dental Screening Form (3300) obtained from the student's doctor or from the health department.
